If gas rate fails would this be done to burner pressure? If not what would be cause?

As in the it didn’t mean the MIs range even with using the 5+ or -10%. What would be the cause for not meeting the range?

Another appliance letting by / pilot on
Bp incorrectly adjusted / out of range
Incorrect pipe sizing / blockage
